Location: Loveland, CO
Status: Full-Time 40/hrs. a week.
As a Specialty Population Case Manager, you will collaborate with agency referral sources including, but not limited to, Probation, Parole, DUI, MAT clinics, and the Department of Human services to support communication between partnering agencies. The position works alongside Outpatient Treatment Providers to submit monthly reporting on client engagement and services provided. In addition, this position tracks data regarding services provided and completes data entry to funding sources. This position is a liaison, connecting treatment providers with referring partners to support client care. The Specialty Populations Case Manager ensures timely responses to referral sources, fostering positive relationships between agencies.

Experiences and Background We Look For
- Required: Bachelor’s degree in related field from accredited college or university; experience with severe and persistent mental illness/substance abuse issues or individuals with similar human-service needs; experience with Word, Excel, and Outlook.
- Preferred: Experience with Trauma-Informed Care
Wage Ranges
- $17.59/HR to $24.62/HR.
